Appointments

Lagoon Medical Centre operates on an appointment-based system. If you require urgent care, we will triage appropriately and advise of potential wait times and any out of pocket costs.


When booking your appointment, we encourage patients to book a longer appointment if your health issue is of a complex nature, or if you have multiple concerns, or if you require more time as in the case of an initial workers compensation consult, a full medical examination, mental health issue or other situation.

Online Bookings

Online bookings are easy and convenient with Hot Doc. They can be made on your mobile phone, tablet or computer. When you use online bookings for the first time you will need to provide some basic details including;

  • Full Name
  • Date of Birth
  • Phone Number
  • Email Address
  • Appointment Type (i.e. standard appointment, long appointment)

You can also select a particular doctor to see all of their available upcoming appointments.

First Visit

As a new patient at Lagoon Medical Centre, on arrival we ask that you complete a new patient registration form.

If you wish to transfer your medical records from your previous surgery, please request a transfer of medical records form, which you can fill out at the practice, (or download here). Our friendly reception team will organise your transfer of medical records. Your previous practice may charge a small fee for this service, we encourage patients to follow up with the practice.

 

Continuity of care

Doctor and patient relationships are important to us as we strive to offer continuous lifelong care for everyone. We believe continuity of care is an important factor in getting to know our patients. When booking your appointment, please request your regular GP. We will do our best to accommodate you. In the situation where doctors are away or unavailable, there are other doctors at the practice who can access your medical records for your treatment.

 

What to bring to your appointment

  • Medicare/Department of Veteran's Affairs/Pension/Health Care card,
  • Photo ID,
  • List of current medication(s), and
  • Results of any investigations (e.g. pathology results, radiology results)
